The following instructions for online appointment booking are for REPEAT PATIENTS ONLY.
If you have never been to our clinic, you are a new patient. Please click here for new patient instructions for online booking.
The “Book an Appointment” button below will take you to a calendar of treatment types where you can book an appointment for follow-up consultation, IV vitamin treatment, microvitamin treatment, or regenerative injection treatment.
To view more information about any one of our appointment types, click on the name of the appointment and you will be able to see information about that appointment.
For your convenience, a variety of follow-up consultation types are available including in-clinic, telephone, and video appointment. Appointments for injection treatment types are in-clinic only.
Once you have decided which appointment type you would like, click PROCEED for that appointment.
The next screen will show available appointment dates and times. If no available times show up, select a different date from the “Browse Dates” dropdown. Select your preferred date and time.
A screen will then appear asking for your name and contact information. Do not fill in that information. Click “ALREADY A CLIENT”.
The next screen will ask for the email address or the cell phone number we have on file for you. Enter either the email address or the mobile phone number that we have on file for you and click “Send Code”.
When you have entered the email address or mobile phone number that we have on file for you, the system will send a 6- digit code either by email or by text, depending on which method you have selected. Enter that 6-digit code in the next screen and click “Verify Code”. The system will fill in your information. Then click “Proceed”.
If, after entering an email address or mobile phone number, the screen displays the following error message:
“We are not able to find a matching client profile. Please confirm you are using the email or phone number you have on file or return to the form to fill in your demographic information”
that means that you have entered an email address or mobile phone number that does not match the one we have on file for you. If you do not recall which email address or mobile phone number is in your clinic file, please call the office to make an appointment over the phone or call and ask us which email address or mobile phone number we have on file for you.
Please do not create a new patient profile.
Once you have entered the 6-digit code using the email address or mobile phone number we have on file for you, and the system has filled your information in for you, and you have clicked “Proceed”, you will then receive a “Request for appointment” email. This email indicates that our staff has received your request for an appointment. During regular business hours, the office will confirm your appointment by sending an email. Once you receive that email your appointment is confirmed.
Please be sure to add:
to your email and telephone contacts list to ensure that you receive appointment confirmation emails, reminder messages, and other electronic communications from us.
Repeat patients who have not been to the clinic in six months or more may need to book a follow up consultation with Dr. Nowazek prior to any injection therapies or other therapies being administered as their health status may have changed. A follow up consultation may be required to determine your current health status and which treatment would be most appropriate based on a current diagnosis by Dr. Nowazek.
Waitlist
We recognize that the waiting time for some appointment types may not meet the needs of patients with advanced or acute (sudden onset and severe) conditions.
To try to meet the needs of patients who require urgent care, we have created a waitlist. The waitlist is a triaged register of patients who need to see the doctor at the earliest possible date.
We sometimes get cancellations of booked appointments. When this occurs, we call patients on the waitlist to see if they would like to have the cancelled time slot. The offer to patients on the waitlist of an earlier appointment could happen on short notice.
If your health situation requires urgent attention and you would like to be on the waitlist for an earlier appointment, please click on the “Waitlist Request” link contained in the appointment confirmation email.
The link will take you to the waitlist form to be completed and returned. Please provide as much detail as possible. The details you provide allow our triage team to assess the urgency of your situation and the care required. Fully detailed information about your health situation could improve your priority standing on the waitlist.
While there are no guarantees, we are often able to secure an earlier appointment for patients on the waitlist.
Please note, you must have a confirmed appointment to register for the waitlist.
